At Supported Coordination our mission is to provide families and participants with clear, detailed information on what services are available to them, and how they can access these services through the NDIS.
We help families and caregivers navigate the NDIS by explaining how it works, exploring long-term support options, and showing how these choices fit into their NDIS goals and plan.
Founded in 2024, Supported Coordination was formed when our director saw a gap within the NDIS system through her own personal experience and how families and caregivers could transition from less formal supports to formal support teams within the home, or to start the journey of exploring Supported living options.
Many adults with disabilities have received long term care in the home from friends or family and have not looked at the formal support options available. We found caregivers and persons with disabilities often struggling to secure long term care plans for their future, due to a lack of information and the complexity of the NDIS system.
