Support Coordination is a CAPACITY BUILDING support which helps you to:
- Understand which supports are available under your current plan.
- Connect you with providers in your area as well as community programs, mainstream programs and other government services.
- Assists you to better navigate the NDIS system.
There are 3 Levels of Support Coordination that can be included in your plan, each of these levels are based on the intensity, level, and types of support that are funded in your current plan.
Helps you to connect with informal supports as well as community funded supports and mainstream programs and connections.
Helps to coordinate your supports; A support coordinator will work with you to help you access and use a variety of supports to increase your capacity, work towards your plan goals and build independence. This can include formal supports, such as support staff and may also include community based supports and other services.
This is a higher, more complex level of support coordination for people who require more specialist support. A specialist coordinator will help you to manage service delivery and any issues that may arise when managing your plan.
